GLP-1 Pens and Needles Compared: What Each FDA Label Says About the Device
GLP-1 pens and needles come in two basic formats, and the FDA labels settle which is which. Wegovy single-dose pens and the Ozempic single-dose prefilled syringe (semaglutide, Novo Nordisk), Zepbound and Mounjaro single-dose pens (tirzepatide, Eli Lilly), Trulicity (dulaglutide, Eli Lilly) and Bydureon BCise (exenatide extended-release, AstraZeneca) are single-use devices with their own built-in needle. The Ozempic multi-dose pen (semaglutide), Saxenda and Victoza (liraglutide), the Wegovy FlexTouch pen and the Zepbound and Mounjaro KwikPens are multi-dose pens that take a separate disposable pen needle — and only the Ozempic pen and Wegovy FlexTouch ship with needles in the carton. Every device is labeled for refrigerator storage, with a room-temperature allowance that ranges from 14 days (Trulicity) to 56 days in use (Ozempic pen, Wegovy FlexTouch). Each statement below names the label, the section and the access date. This is a transcription of public labeling, not medical advice.

The short answer
GLP-1 pens and needles split into two formats on the FDA labels: single-dose devices with a built-in needle (Wegovy, Zepbound, Mounjaro, Trulicity and Bydureon BCise pens, and the Ozempic single-dose prefilled syringe) and multi-dose pens that take a separate pen needle (the Ozempic pen, Wegovy FlexTouch, Saxenda, Victoza, Zepbound and Mounjaro KwikPens). Only the Ozempic pen and Wegovy FlexTouch include NovoFine Plus needles. All are refrigerated before use; in-use windows run 30 to 56 days.

Key takeaways
- Single-dose, needle-included, needle built in: the Wegovy single-dose pen, the Ozempic single-dose prefilled syringe, the Zepbound and Mounjaro single-dose pens, the Trulicity single-dose pen and the Bydureon BCise autoinjector, per each product's Instructions for Use (via DailyMed and FDA accessdata, accessed 14 September 2026).
- Multi-dose, separate needle required: the Ozempic multi-dose pen, Wegovy FlexTouch, Saxenda, Victoza, and the Zepbound and Mounjaro single-patient-use KwikPens. The Ozempic pen and Wegovy FlexTouch include NovoFine Plus 32G 4 mm needles; the Saxenda IFU states pen needles are not included; the Zepbound and Mounjaro KwikPen IFUs list a KwikPen-compatible pen needle as not included.
- Before first use, every one of these devices is labeled for refrigerator storage at 2°C to 8°C (36°F to 46°F); seven of the eight labels add do-not-freeze wording, and Bydureon BCise adds that it must be stored flat. The unopened room-temperature windows differ: Trulicity 14 days, Zepbound and Mounjaro single-dose pens 21 days, Wegovy single-dose pens and Ozempic syringes 28 days, Bydureon BCise 4 weeks, and the Victoza IFU gives 30 days for a pen left out of the refrigerator by mistake.
- After first use, the multi-dose pens run on a clock the label states: 56 days for Ozempic and Wegovy FlexTouch, 30 days for Saxenda and Victoza, and 30 days or four weekly doses for the Zepbound and Mounjaro KwikPens, at room temperature up to 30°C (86°F) or refrigerated.
- This page publishes no dose figures, no titration and no instruction to change anything. It describes devices and storage as the labels state them; how you use yours is set by your prescriber and the IFU that came with the box.
Six presentations need no needle from you
The Wegovy single-dose pen IFU states the needle is covered by the needle cover and will not be seen. The Ozempic single-dose prefilled syringe IFU walks through inserting the syringe's own needle, with no pen needle to attach. The Zepbound and Mounjaro single-dose pen IFUs describe a gray base cap over a pre-attached needle. The Trulicity IFU describes a disposable, prefilled single-dose pen whose needle retracts after the second click. The Bydureon BCise IFU states the needle is hidden and covered by the orange cap (all accessed 14 September 2026).
Two multi-dose pens ship with needles, four do not
The Ozempic pen's Section 16 carton table lists NovoFine Plus needles packed with every pen, and the Wegovy FlexTouch IFU says NovoFine Plus 32G 4 mm needles are included. The Saxenda IFU states plainly that pen needles are not included. The Victoza IFU refers you to your provider or pharmacist for needles, and the Zepbound and Mounjaro KwikPen IFUs list the pen needle as not included.
Room-temperature windows are per device, not per drug
The same molecule can carry two different windows. A Wegovy single-dose pen may sit at 8°C to 30°C for up to 28 days before the cap is removed; the Wegovy FlexTouch multi-dose pen is labeled for 56 days after first use. A Zepbound single-dose pen is labeled for a total of 21 days unrefrigerated; a Zepbound KwikPen for 30 days once opened. The device determines the number.
Every figure names its label and date
Each storage window on this page was read from Section 16, How Supplied/Storage and Handling, or from the Instructions for Use of the named product, on DailyMed or FDA accessdata, accessed 14 September 2026. Labels are revised; the current label always wins over this page.
Which GLP-1 pens include a needle, product by product
The question people actually bring to a pharmacy counter is simple: do I need to buy pen needles for this? The FDA labels answer it device by device, and the answer depends on the device far more than on the molecule.
Needle already attached, nothing to buy: the Wegovy single-dose pen and single-dose syringe (the Wegovy label, Section 16, describes the syringes as having integrated needles); the Zepbound and Mounjaro single-dose pens; the Trulicity single-dose pen; the Bydureon BCise autoinjector; and the Ozempic single-dose prefilled syringe, whose IFU walks through inserting the syringe's own needle. All are single-use and are discarded after one injection.
Separate pen needle required, needles included in the carton: the Ozempic multi-dose pen, whose Section 16 carton table lists NovoFine Plus needles with each pen, and the Wegovy FlexTouch single-patient-use pen, whose IFU states NovoFine Plus 32G 4 mm disposable needles are included.
Separate pen needle required, needles not included: the Saxenda pen (its IFU says pen needles are not included and the pen is made for disposable needles up to 8 mm long), the Victoza pen (its IFU directs you to your healthcare provider or pharmacist for information about needles), and the Zepbound and Mounjaro single-patient-use KwikPens (each IFU lists a KwikPen-compatible pen needle as not included). The Zepbound and Mounjaro vials also require a separately supplied syringe and needle. All read via DailyMed, accessed 14 September 2026.
Where a separate needle is used, every one of these labels says the same two things: use a new needle for each injection, and never share the pen with another person even if the needle is changed, because of the risk of transmitting blood-borne pathogens.

Single-dose autoinjectors: Wegovy, Zepbound, Mounjaro, Trulicity, Bydureon BCise
Five products in this set are fixed-dose, single-use devices. You do not dial anything, attach anything or prime anything. The label language for each is specific enough to quote.
Wegovy single-dose pen (Novo Nordisk; label revised 06/2026): the IFU states the needle is covered by the needle cover and will not be seen, that the injection starts when the needle cover is pressed firmly against the skin, and that the needle cover locks when the pen is removed. Section 16 says to discard the pen or syringe after use.
Zepbound and Mounjaro single-dose pens (Eli Lilly; both labels revised 8/2026): the IFU describes a gray base cap that is pulled straight off and never replaced, because doing so could damage the needle, then a clear base placed on the skin, a lock ring turned to unlock, and an injection button. The gray plunger becoming visible signals the injection is complete.
Trulicity single-dose pen (Eli Lilly; label revised 3/2026): the IFU calls it a disposable, prefilled medicine delivery device, each pen containing one dose and used one time. After the base cap comes off, a clear base is placed on the skin, the lock ring is turned, the green button is pressed, and a second click marks the needle starting to retract in about 5 to 10 seconds.
Bydureon BCise autoinjector (AstraZeneca; label revised 05/2025, via FDA accessdata): the IFU states it is a single-use, fixed-dose autoinjector that automatically injects, that the needle is hidden before, during and after use, and that a green shield covers the needle once the orange cap is unscrewed. It is the only device here that must be shaken hard for at least 15 seconds and rested flat for 15 minutes before injection, because it is a suspension rather than a solution.
For anyone keeping records, the practical difference is that a single-dose device has no in-use clock. The only storage question is how long it has been out of the refrigerator before the cap came off, which the next sections cover.
Multi-dose pens with separate needles: Ozempic, Wegovy FlexTouch, Saxenda, Victoza, and the tirzepatide KwikPens
Six presentations in this set are multi-dose pens holding several doses in one cartridge. They share a routine the labels describe in near-identical words: attach a new disposable pen needle, check flow or prime as the IFU describes, dial the dose, inject, then remove and discard the needle and store the pen with the cap on and no needle attached.
Ozempic (Novo Nordisk; label revised 05/2026) is described in Section 16 as a single-patient-use pen with multiple weekly doses. The IFU states NovoFine Plus 32G 4 mm disposable needles are included, and that NovoFine Plus, NovoFine or other compatible needles may be used if more are needed. Section 16 adds that the pen must be stored without an injection needle attached and that a new needle is used for every injection.
Wegovy FlexTouch (Novo Nordisk; label revised 06/2026) is the newer multi-dose presentation of semaglutide for weight management, listed in Section 16 as a single-patient-use pen with four weekly doses. Its IFU describes a prefilled, dial-a-dose pen, states NovoFine Plus 32G 4 mm needles are included, and says compatible disposable needles up to 8 mm in length can be used. The label notes it is not recommended for self-administration by those with visual impairment unless the provider determines otherwise.
Saxenda and Victoza (Novo Nordisk; Saxenda Instructions for Use revised 06/2026, Victoza label revised 10/2025) are both prefilled, dial-a-dose, single-patient-use pens. The Saxenda IFU is explicit that pen needles are not included and that the pen is made for disposable needles up to 8 mm. The Victoza IFU tells the reader to ask a provider or pharmacist about needles. Both Section 16 texts instruct storing the pen without a needle attached to reduce contamination, infection and leakage and to preserve dosing accuracy.
Zepbound and Mounjaro KwikPens (Eli Lilly; labels revised 8/2026) are single-patient-use pens holding four weekly doses, with extra volume for priming. Each IFU lists a KwikPen-compatible pen needle as not included, requires priming before every weekly injection, and describes changing the needle if no drop appears after repeated priming. The same labels list multi-dose vials, which need a separate syringe and needle, and single-dose vials, which do too.
If your pen ran out before you expected it to, the reasons usually involve priming volume and dose counting rather than a device fault; that situation has its own page at GLP-1 pen ran out early.
Storage before first use, as each Section 16 states it
Every injectable GLP-1 device in this set is labeled for refrigerator storage at 2°C to 8°C (36°F to 46°F) before first use. Seven of the eight labels add do-not-freeze and do-not-use-if-frozen wording; the Bydureon BCise Section 16 instead states that the autoinjector must be stored flat, in the original package, protected from light. The instruction not to store the product directly adjacent to the refrigerator cooling element appears only on the four Novo Nordisk labels (Wegovy, Ozempic, Saxenda and Victoza). Wegovy, Ozempic, Zepbound and Mounjaro state original-carton storage to protect from light; Trulicity Section 16.2 says storage in the original carton is recommended. The differences are in the unopened room-temperature allowance, which is the number people most often need when a delivery sits on a porch or a fridge fails.
Wegovy single-dose pen and syringe: Section 16 states that, if needed and prior to cap removal, the pen can be kept from 8°C to 30°C (46°F to 86°F) up to 28 days. The Ozempic single-dose prefilled syringe carries the same 28-day, 8°C to 30°C wording.
Zepbound and Mounjaro single-dose pens and single-dose vials: Section 16.2 states each can be stored unrefrigerated at temperatures not to exceed 30°C (86°F) for up to a total of 21 days, after which it is discarded. The unopened Zepbound or Mounjaro KwikPen or multi-dose vial is labeled for use until the expiration date if refrigerated, or 30 days if stored at room temperature up to 30°C.
Trulicity: Section 16.2 states each single-dose pen can be kept at room temperature not to exceed 86°F (30°C) for a total of 14 days.
Bydureon BCise: Section 16 states it can be kept at room temperature not to exceed 86°F (30°C) for no more than a total of 4 weeks, and that it must be stored flat.
Ozempic pen, Saxenda, Victoza and Wegovy FlexTouch: the Section 16 text for each says to refrigerate prior to first use and gives its room-temperature window as an after-first-use figure. The Ozempic storage table puts it in one line: prior to first use, refrigerated until the expiration date; after first use, 56 days. Victoza is the exception, and the figure sits in its Instructions for Use rather than in Section 16: the Victoza IFU states that if the pen is stored outside of refrigeration by mistake prior to first use, it should be used or thrown away within 30 days. Neither the Ozempic, Saxenda nor Wegovy FlexTouch label states a separate unopened room-temperature window in the text read for this page, so this page does not invent one.
All of these are label statements, not permissions. A pen that has been warm for an unknown time is a question for the pharmacist, and the practical decision tree lives at GLP-1 left out of the fridge. For trips, see keeping a GLP-1 cold while traveling.
Storage after first use: the in-use clock on multi-dose pens
Once a multi-dose pen has been used the first time, the label starts a clock that runs regardless of how much medicine is left. These are the figures, each from the product's Section 16 or IFU via DailyMed, accessed 14 September 2026.
Ozempic: after first use, the pen can be stored for 56 days at controlled room temperature 15°C to 30°C (59°F to 86°F) or in a refrigerator 2°C to 8°C (36°F to 46°F). The IFU adds that the pen is thrown away after 56 days even if it still has medicine in it.
Wegovy FlexTouch: after first use, store between 59°F to 86°F (15°C to 30°C) or in the refrigerator for up to 56 days; the IFU says to throw it away if it has been frozen, exposed to light or temperatures above 86°F, or out of the refrigerator for 56 days or longer.
Saxenda and Victoza: after first use, each pen can be stored for 30 days at controlled room temperature 59°F to 86°F (15°C to 30°C) or in a refrigerator 36°F to 46°F (2°C to 8°C), and is thrown away after 30 days.
Zepbound and Mounjaro KwikPens and multi-dose vials: once opened, store in the original carton in the refrigerator or at room temperature up to 30°C (86°F), and throw away after a total of 30 days at room temperature, 30 days after first use, or after taking four weekly doses, even if medicine remains.
Three handling rules appear on every one of these labels: keep the pen cap on when not in use, protect from excessive heat and sunlight, and store the pen with no needle attached. Single-dose devices have no in-use clock at all because they are discarded after the one injection.
The number that matters for a record is the first-use date. A pen started on the first of the month has a label-defined last day, and writing that day down is far more reliable than remembering it. Where a device is injected is a separate topic covered in how to rotate GLP-1 injection sites.
Disposal, and what is worth logging about the device
Every IFU in this set ends the same way: put the used pen, syringe or pen needle in an FDA-cleared sharps disposal container right away after use, never in the household trash, and follow state or local rules for the container itself. The Ozempic, Zepbound and Trulicity IFUs point to the FDA's safe sharps disposal page for local guidance. Single-dose pens go into the container whole; multi-dose pens contribute a needle per injection and the pen itself at the end of its clock. The practical options, including mail-back programs and what to do while traveling, are on what to do with used GLP-1 pens. Flying with the devices themselves is covered at flying with a GLP-1.
Device details are the kind of record that seems unnecessary until it is needed. When a pharmacy switches you from a single-dose pen to a FlexTouch or KwikPen, the Wegovy, Zepbound and Mounjaro labels all instruct that patients receive training for the new presentation and read its own IFU. A log that records which presentation you were dispensed, the date a multi-dose pen was first used, and any day a device spent outside the refrigerator gives your pharmacist the facts they need to answer the storage question accurately.
GLP-1 Tracker, a free iPhone app, is built for that layer and no more: it logs each dose with its date and site, holds a note per entry where a pen's first-use date or a fridge lapse can be written down, and keeps weight and meals on the same timeline. It does not decide whether a warm pen is usable, does not compute a dose, and does not replace the IFU in the box. Educational and organizational only.
Frequently asked questions
Which GLP-1 pens come with needles included?
Two multi-dose pens ship with needles: the Ozempic multi-dose pen, whose Section 16 carton table lists NovoFine Plus needles with each pen, and the Wegovy FlexTouch pen, whose Instructions for Use state NovoFine Plus 32G 4 mm disposable needles are included (both via DailyMed, accessed 14 September 2026). Single-dose devices such as the Wegovy, Zepbound, Mounjaro and Trulicity pens, the Bydureon BCise autoinjector and the Ozempic single-dose prefilled syringe have the needle built in, so nothing separate is needed.
Do Saxenda and Victoza come with pen needles?
No. The Saxenda Instructions for Use state that pen needles are not included with the pen and that it is made to be used with disposable needles up to 8 mm in length. The Victoza Instructions for Use direct patients to their healthcare provider or pharmacist for information about needles. Both labels, read via DailyMed accessed 14 September 2026, instruct using a new needle for every injection and storing the pen without a needle attached.
Do the Zepbound and Mounjaro KwikPens include pen needles?
No. The Zepbound and Mounjaro KwikPen Instructions for Use, in the labels revised 8/2026 and read via DailyMed accessed 14 September 2026, list a KwikPen-compatible pen needle as not included. The same labels' single-dose pens have a pre-attached needle under a gray base cap, and the single-dose and multi-dose vials require a separately supplied syringe and needle. Which presentation you receive is decided by your prescriber and pharmacy, not by this page.
How long can an unopened GLP-1 pen stay out of the fridge?
It depends on the device, and each label states its own figure. The Wegovy single-dose pen and the Ozempic single-dose syringe are labeled for up to 28 days at 8°C to 30°C before cap removal. Zepbound and Mounjaro single-dose pens are labeled for a total of 21 days unrefrigerated, not exceeding 30°C. Trulicity is labeled for a total of 14 days at room temperature, and Bydureon BCise for no more than a total of 4 weeks. The Victoza Instructions for Use states that a pen stored outside of refrigeration by mistake before first use should be used or thrown away within 30 days. All figures are from Section 16 or the Instructions for Use of each label, accessed 14 September 2026. A pen with an uncertain history is a question for your pharmacist.
How long can an Ozempic or Wegovy FlexTouch pen be used after the first injection?
Both labels state 56 days. Ozempic's Section 16 says that after first use the pen can be stored for 56 days at controlled room temperature 15°C to 30°C (59°F to 86°F) or in a refrigerator at 2°C to 8°C, and the Instructions for Use say to throw the pen away after 56 days even if medicine remains. The Wegovy FlexTouch Instructions for Use give the same 56-day window after first use at room temperature or refrigerated. Both read via DailyMed, accessed 14 September 2026.
Why must a multi-dose pen be stored without the needle attached?
The Saxenda and Victoza labels give the reason in Section 16: storing the pen without an injection needle attached reduces the potential for contamination, infection and leakage while also ensuring dosing accuracy. The Ozempic and Wegovy FlexTouch labels carry the same instruction and add that a new needle is used for each injection. Every pen label in this set also states that a pen must never be shared between patients, even if the needle is changed.
Is Bydureon BCise different from the other single-dose pens?
Yes, in handling. The Bydureon BCise label, read via FDA accessdata accessed 14 September 2026, states the autoinjector must be stored flat, rested flat at room temperature for 15 minutes before mixing, and shaken hard for at least 15 seconds until the suspension is evenly mixed. The needle is hidden and covered by the orange cap and then by a green shield. It can be kept at room temperature not exceeding 86°F for no more than a total of 4 weeks.
Does this page tell me which pen or needle to use?
No. It transcribes what each FDA label states about its own device, needle supply and storage, with the section and access date, so the source of a claim can be checked. It does not recommend a product, a needle length, a dose or any change to how you inject. Those decisions belong to the prescriber and pharmacist who know your situation, and the Instructions for Use that came with your specific presentation take precedence over anything summarized here.
